All-on-4 vs. All-on-6: It’s Not Just About Numbers, It’s About Engineering

When searching for full-mouth teeth restoration, you will constantly come across two major terms: All-on-4 and All-on-6 dental implants. A very common misconception among patients is that All-on-6 is simply an "upgraded" or more expensive version of All-on-4. In reality, the choice between four or six implants is a purely medical and biomechanical decision made by your oral surgeon.

1. Jaw Anatomy and Bone Density

The primary factor in choosing the number of implants is the volume and quality of your remaining jawbone:

  • Why All-on-4? This technique was specifically engineered for patients with significant bone loss, especially in the back (posterior) areas of the jaw. By tilting the two back implants at a 45-degree angle, we can utilize the stronger bone available at the front of the mouth, often completely avoiding the need for expensive bone grafts or sinus lifts.
  • Why All-on-6? If you have sufficient bone density and volume extending toward the back of your jaw, our surgeons will likely recommend All-on-6. Placing two additional implants in the back provides an incredibly stable, continuous foundation.

2. Distribution of Bite Force (The Physics of Chewing)

Think of dental implants like the pillars supporting a bridge. The more pillars you have, the more evenly the weight is distributed:

  • All-on-4 creates a reliable, sturdy tripod-like suspension that easily handles everyday chewing forces for most patients.
  • All-on-6 distributes your bite force across six points instead of four. If you have a naturally strong jaw, large facial structure, or a habit of grinding your teeth (bruxism), the two extra implants act as a safety net, reducing the mechanical stress on each individual implant and the jawbone.

3. Upper vs. Lower Jaw Mechanics

Your upper and lower jaws have completely different bone structures. The lower jawbone is naturally much denser and harder, meaning an All-on-4 system usually offers absolute stability there. The upper jawbone, however, is softer and more porous. This is why our clinical team frequently prefers an All-on-6 configuration for the upper arch to maximize surface area contact and ensure long-term success.

Ultimately, neither option is universally "better" than the other. The perfect choice depends entirely on a detailed analysis of your 3D CT scan, your chewing habits, and your unique anatomy.
